In Diablo 4 WT4, Malignant Hearts drop mainly from Tree of Whispers caches and are the bottleneck part for summoning Echo of Varshan, so stack caches fast, then run Varshan kills in batches for top-tier loot.
Malignant Hearts are back in Diablo 4, but not in the old "stick it in a ring" way. Now they're a straight-up summoning material, and if you're trying to keep up with endgame boss rotations you'll feel that shortage fast. If your time's tight, some players even top up gold or snag gearing help through EZNPC, then spend their actual play sessions doing the boring part: farming the parts needed for Echo of Varshan instead of messing around in half-finished content.
Get to Torment first
Don't waste energy hunting Hearts before World Tier 4. They're Torment-only, so everything before that is basically prep: capstone, resistances sorted, and a build that can clear without face-planting. You'll notice pretty quickly that "I can survive" isn't the same as "I can farm." If elites are taking forever, it's not just slower XP—your material income tanks too. Aim for a setup where you can sprint from pack to pack and keep your cooldowns rolling, even if it means dropping a bit of damage for consistency.
The Tree of Whispers loop
Your bread and butter is the Tree of Whispers. Turn in Grim Favors, open caches, repeat. The easiest wins are Whisper dungeons because they're a clean five favors in one run, and you can chain them without thinking. If the map has a tight cluster of quick objectives—cellars, kill targets, simple events—hit those first. Legion events and world bosses fit nicely too since they're quick, social, and they push you back toward more favors. Keep it simple: speed clears, short travel time, and no heroics.
RNG is the real boss
Here's the annoying bit: Hearts feel streaky. You'll stack piles of other body parts and still go ages without seeing the one thing you actually need. That's normal. Your stash ends up looking like a horror prop department while your Heart count sits at zero. Don't assume it's bugged, and don't tilt and start doing slower content "just in case." More caches per hour is still the best way to beat the dry spells, even when it doesn't feel like it.
Spend hearts like they matter
Once you've got the full set, head to the Malignant Burrow by the Tree and summon Varshan at the altar. The mistake a lot of people make is burning a Heart the second they get it, then leaving after one mediocre drop. Stockpile instead, then do a proper session: multiple summons, same rhythm, less downtime.
